Coaching Information

Please report your weekly score to teamsideline every Saturday after your game.

To be a coach or assistant coach you will need to fill out the coach's application, fill out the CYSA disclosure form, provide a 1x1 picture (no sunglasses or hat) and be fingerprinted with the club. Links are below for all the forms.

Note: The Club form must be printed first then filled out. The CYSA form must be completed electronically prior to printing the forms. Please follow all the directions given on the site.

Once the forms are completed attach a 1 by 1 inch picture (no hats or sun glasses) to the forms and mail them to the club.

Fingerprinting- Live Scan will be at our coaches meeting on July 17th.

All coaches and assistant coaches need to be fingerprinted. If you were fingerprinted last year by the soccer club you do not need to do this again. This is for any new coach or assistant or anyone who did not do this last year.
