Fingerprinting- Live Scan will be at our coaches meeting on July 15.
Practices start in August, once you have decided what time and day you want to practice please email your age coordinator with you request. Please indicate your team name too. Go to the field section and click practices to see what is available. Below are the fields and please pick the field that is for your age group. We will not be practicing at Mather Heights this year, you can still practice at Independence Park. No goal posts at Independence. Lincoln Village Park is close to Mather and practices can be held there. Maps of the fields are on the website under fields.
U6 - Gold River School, Independence Park & Argonaut
U8 - Gold Station, Sonoma, Independence Park & Sandpiper park, Hagan Park (small field)
U10 - Tuscany and Prospect Hill Park, Hagan Park
U12 - Hagan & Sunriver
U14 - Mather Independence Park
U16/U19 - Dave Roberts/Riverview
8 to U19 Coaches, please report your weekly score to teamsideline every Saturday after your game.
To be a coach or assistant coach you will need to fill out the coach's application, fill out the CYSA disclosure form, provide a 1x1 picture (no sunglasses or hat) and be fingerprinted with the club. Links are below for all the forms.
If there is an injury during practice or a game please click on the link below and fill out the incident report.
CYSA CASE REPORT (Fill-In Form)
Note: The CYSA CASE REPORT must be completed and submitted into the CYSA State Office within ninety (90) days from the date of injury
Note: The Club form must be printed first then filled out. The CYSA form must be done online only. Please follow all the directions given on the site.
Once the forms are completed attach a 1 by 1 inch picture (no hats or sun glasses) to the form and mail them to the club. If you coached last year we do not need another picture. We still have it on file.
